Registration Documents

Newly admitted students are required to bring the following documents to the UFAZ Academic Building to complete the registration process:

• Curriculum Vitae (CV) – original and one copy (handwritten in Azerbaijani);
• Two copies of the national ID card;
• Medical certificate (Form 086) and one copy;
• Original diploma and transcript or copies thereof (students who have not yet received their diploma must provide a signed and stamped certificate from their university);
• Four passport-sized photographs (3×4 cm);
• Statement of the DİM examination result and one copy;
• A copy of the document confirming the status of applicants who are internally displaced persons (IDPs), children of war veterans with disabilities, children deprived of parental care, or members of martyrs' families;
• For male students: three copies of the military registration document.
